Thank you for allowing your minor child to participate in our church functions! By checking below, you are indicating to the leadership of Bible Baptist Church that you will adhere to the following health and safety standards set by Bible Baptist Church:
• If your minor child is sick, shows symptoms of being sick, has a fever, or has recently been exposed (within the last 10 days) to any contagious illness or virus, you will agree to NOT bring him/her to any activity of Bible Baptist Church until the 10 day period has passed. The health and well-being of all children and adults is of utmost importance to us. Therefore, if we observe that your minor child is sick, or has a fever, or if he/she refuses to submit to our health and safety procedures, we will separate your child from the others in his/her group, and will ask you to come and get him/her immediately. Thanks so much for your cooperation and understanding!
• Please help us by making your child aware of the following directives during church activities and while in church-owned vehicles. If your child disregards these directives, the privilege of attending church activities may be suspended until such time as there is a display of willingness to comply.

• As the parent/guardian, you acknowledge and accept the risks of physical injury associated with participation in any and all activities of Bible Baptist Church, including church-provided transportation to and from those activities. Except for gross negligence on the part of personnel of Bible Baptist Church, the participant (or parent / guardian) accepts personal, financial responsibility for any bodily or personal injuries while participant is involved in activities of Bible Baptist Church—including transportation related to those activities. Further, the participant (or parent/guardian) promises to hold harmless Bible Baptist Church, its representatives, or workers for any injury sustained while participant is involved in activities of Bible Baptist Church—including church-provided transportation associated with those activities. If a dispute over this agreement or any claim for damages arises, the participant (or parent/guardian) agrees to resolve the matter through a mutually acceptable arbitration process.
